Hamm Clinic is committed to protecting your privacy. This SMS Message Privacy Policy governs how we collect and use information about you in relation to Hamm Clinic text messaging use, which we make available to you through a third-party service provider.
By using this messaging service, you agree to the terms of this policy. Hamm Clinic reserves the right to modify or change this policy at any time with or without prior notice to you. The date of the last update will be posted at the top of this policy for your convenience. This policy, and any changes, are effective as soon as posted and supersede any prior policies. Your continued use of the message service following the posting of any changes to the Policy constitutes your full acceptance of those changes.
Collection, Use and Retention of Information
Through your use of the messaging service, we will receive Personal Information through our third-party service provider. “Personal Information” is information that individually identifies you, such as your mobile phone number you provided when signing up for the messaging service, your name, and/or any other information that you choose to include in messages you send through the messaging service.
Hamm Clinic will use your information for the sole purpose of communication regarding appointment scheduling, cancellations and/or reminders.
Hamm Clinic may collect and retain any text messages sent between you and staff and will follow and adhere to all HIPAA (Health Insurance Portability and Accountability Act) policies and procedures. SMS consent and phone numbers will not be shared with third-parties or affiliates for marketing purposes.
Risks of Text Message Communication
Hamm Clinic wants to inform you of the risks of communication via text messaging, which can include but are not limited to unauthorized access by a third-party. Text messages may accidentally be sent to the wrong number by either party, messages may be viewed on your mobile device by others if you have not secured it with a password/passcode or if you have chosen to share your device & passcode with others, or if your device is owned by your employer. Please take care to restrict access to your text messages on your mobile device. Hamm Clinic will only text to the mobile number you have provided to us for text messaging purposes.
Terms & Conditions of Text Messaging (SMS)
- You may OPT-IN to Text Messaging with your clinician if you complete and return a SMS Text Message OPT-IN form.
- Text messages may include appointment reminders and messages about scheduling changes from your providers and care team.
- Text messaging is not for communication about your mental health or other clinical/medical information. Please call us to discuss any clinical concerns or matters with your clinician at (651) 224-0614.
- Text messages sent or received may be included in your medical health record.
- Message and Data Rates may apply.
- Text message frequency may vary depending on your individual circumstances and usage with your clinical provider.
- Not all Hamm Clinic clinicians use Text Messaging (SMS) Services, and you will not be allowed to opt-in if your individual clinician does not use the service.
- Text messages are not checked outside of normal business hours and days. Although we try to respond to messages in a timely manner, we cannot guarantee that we will respond in a certain timeframe. Please call us if you have time sensitive needs at (651) 224-0614.
- If you change your phone number, please call us at (651) 224-0614 to make sure we update your phone number in our system.
- If you wish to stop Text Messaging communications with Hamm Clinic at any time, you can OPT-OUT by texting your clinician “STOP”, or by calling us at (651) 224-0614, emailing us at info@hammclinic.org, or in writing to Hamm Clinic at 10 River Park Plaza, Suite 710, St. Paul, MN 55107.
